New Patient Information, Forms, & FAQs
We ask that our new patients arrive fifteen minutes prior to their scheduled appointment time in order to complete the new patient paperwork. If you prefer, you may also print out the forms below, complete them at home, and bring them along to your appointment. Prior to your first visit with us, we also ask that you contact your previous dentist and request that any current x-rays be forwarded to our office.
During your first visit, we will review your medical and dental history, provide you with a comprehensive oral exam by one of our wonderful dentists, and our staff may take a few digital images (x-rays). We allow time during this appointment for you to ask questions and discuss your treatment needs, so do not hesitate to address any concerns that you may have.

Does your office see children?
Yes! We encourage parents to talk to their children about going to the dentist. Some children may need to observe an appointment first to ease them into their own appointment. You may bring your child to your cleaning appointment so that they may observe, and we can give him or her "a ride in the chair" and count his or her teeth if they are comfortable. Our hygienists will go over all the tools used for cleanings and will explain what your child can expect. We recommend that children have their first dental visit around the age of three. If, however, you have any concerns about your child's dental health before that time, please feel free to contact us.
What if you do not participate with my insurance?
We do have patients with whose insurance we do not participate. In these instances, patients pay at the time of their visit for services received that day. We will then submit the claim on the patient's behalf, and he or she will be reimbursed by the insurance company. If you have any questions regarding our participation with your insurance, you may refer to the list of those with which we participate under the "Our Practice" tab on our website. The HR department of your employer can provide you with specific information regarding what plan or coverage you may have.
How do I schedule my first appointment?
To schedule your inital appointment, please call our office at (717) 795-9392 during our business hours. Please be sure to inform us of any medical conditions or concerns, especially a history of heart conditions and joint replacements.
What happens if I need to cancel/reschedule my appointment?
It is important to us that we provide optimal dental care to all of our patients. In order for us to accomplish this goal, we rely on you as the patient to be aware of your scheduled appointments. As a courtesy to you, we will contact you to remind you of upcoming appointments. If we are unable to reach you directly and must leave a message or send an email, we will assume that you plan to keep your appointment unless we hear from you.
We do require a 24-hour notice in order to cancel an appointment, enabling us to offer that time to another patient. However, we realize that life happens--children get sick, people sometimes forget, and that emergencies arise. As soon as you know that you will be unable to keep your appointment, please call and notify us--even late at night. We do apply a fee of $35.00 for failed hygiene appointments and a fee of $65.00 for failed appointments with the doctors.
